New Patients
Registering at the Practice
The practice accepts all requests to join the patient list without discrimination. It is practice policy to refuse patients who reside outside the practice area and those who have a history of violence towards practice staff or have been previously removed from this list.
Patient Registration Forms
Before being accepted as a patient at this practice two forms need to be
completed and submitted, the GMS1 registration form and the new
patient questionnaire. Please download and complete the new patient
questionnaire and bring it to the surgery where you can complete the GMS1 form.
